Using Questionnaires in CAMHS:

At the first CAMHS appointment we will ask you and your child to fill out some questionnaires. These ask about how your child is feeling and what things they are finding difficult or easy at the moment. The information you give us on these forms helps us understand more about your child’s specific difficulties and what they need support with.

If you come to CAMHS for more appointments, your clinician will discuss using questionnaires with you and your child on a regular basis. These will track your concerns and how much progress you and your child feel is being made. Your clinician will check these regularly to make sure you are receiving the best possible support.

You are always allowed to say if you would prefer not to use a questionnaire.

Are the questionnaires used for anything else?

With your permission we also use the information you provide on questionnaires to understand how we are doing as a service. We anonymous the answers and enter them into a computer. We then look at them together with the answers from all the other families coming to CAMHS. This means we can understand what we do really well and what things we need to improve. This helps us to make sure we are always providing the best possible service to children, young people and their families.

You can read more about this and see examples of the questionnaires we use here: http://www.corc.uk.net/about/young-people-and-families/.

What if I don’t want you to use my information in this way?

We will always ask your permission first before looking at your questionnaire answers in this way. We will give you an information sheet explaining exactly how we use the questionnaires and there is an option for you to tick if you are happy to consent. You are allowed to decline and you can withdraw your consent at any time. Our clinicians will always be happy to answer any questions you have about using questionnaires at CAMHS.
